The authorization of a parent or guardian is required for registration in FitSpirit. The authorization form will be emailed to your parent or guardian. It is therefore important to accurately provide the information requested below. Ask your FitSpirit Program Leader how to proceed if you don't know your parent/guardian's email address. You can also register at home with your parent/guardian present.
